Volunteer Application

1. Volunteer Applicant should bring a current driver license and social security card.
2. Volunteer Applicant shall complete an application.
3. Completed application shall be reviewed by Administrative Assistant.
4. If accepted for volunteer work, information will be sent to the Polk County Sheriff's Office for a background check.
5. Upon receipt of a favorable background check, the volunteer can begin under the sole supervision of a staff member. No volunteer is to have sole supervision of the clients.
